This is where the request form opens

On the practice's live site, this page opens the appointment request form in its practice software, the same system that holds your pet's records and sends reminders.

Glenrose Animal Hospital is a fictional practice, built as a portfolio case study, so no request form is connected here and nothing is sent anywhere. To show how the path works, this page stands in for it.

To book now, call the front desk at (509) 555-0142.

What happens after you send a request

  1. 1

    You tell us the basics.

Your name and phone number, your pet's name, whether it's a dog or a cat, and what the visit is for.

  2. 2

The front desk calls you back.

They find a time that works, and tell you which doctor you will see. New clients usually start with Dr. Whitcomb.

  3. 3

You come in with your questions.

Bring your records and your list. You leave with a plan, and a written estimate for anything beyond the exam.

If your pet is sick and it cannot wait for a callback, call (509) 555-0142 during open hours. Outside our hours, take your pet to your nearest emergency animal clinic.
